This page is to book a D4 Driver Medical appointment in clinic. Please be aware that payment may be required during the booking process. You will also need to complete a consent form prior to the appointment. This will be sent to you in an email following booking completion
Please can you bring to your appointment a copy of your NHS GP Medical Record Summary Sheet. Please be aware if you wear glasses, you will also need to get an Optician to sign off the vision section of the D4 form (this can be done before or after your appointment with The IGP).
