This short questionnaire is based on the World Health Organization (WHO) Adult ADHD Self-Report Scale. It can help you understand whether your current symptoms may be consistent with Attention Deficit/Hyperactivity Disorder (ADHD) in adults.

Important: This tool is for adults aged 18+ and for information only. It does not provide a diagnosis. A formal diagnosis of ADHD can only be made by a suitably qualified healthcare professional after a comprehensive clinical assessment.

ADHD Assessment Questionnaire

Please answer these questions thinking about the last 6 months.

1. How often do you have trouble wrapping up the final details of a project, once the challenging parts have been done? *

2. How often do you have difficulty getting things in order when you have to do a task that requires organization? *

3. How often do you have problems remembering appointments or obligations? *

4. When you have a task that requires a lot of thought, how often do you avoid or delay getting started? *

5. How often do you fidget or squirm with your hands or feet when you have to sit down for a long time? *

6. How often do you feel overly active and compelled to do things, like you were driven by a motor? *

ADHD Assessment Information

This questionnaire is a screening tool. A positive screen with a score of 4 or more means your answers are similar to people who are later diagnosed with ADHD, but it is not a diagnosis on its own. A formal diagnosis always requires a full clinical assessment.

If you are concerned about ADHD or your mental health, you can book an appointment with an iGP doctor or speak to your NHS GP. If you feel unable to cope, or have thoughts of harming yourself, please seek urgent help via NHS 111, your local NHS service, or your nearest Emergency Department.
