ADHD Pre-Assessment Questionnaire

We understand that many adults live with undiagnosed ADHD, often without realising that the challenges they face each day may be linked to this condition. Recognising this, completing this simple self-screening questionnaire will help us identify those who may benefit from further support.

Only a qualified clinician can make a formal ADHD diagnosis. However, the questionnaire is a good indicator of whether you may benefit from a full ADHD assessment.

Frequently Asked Questions

What is an ADHD assessment?

An ADHD assessment is a detailed evaluation carried out by a qualified clinician to determine whether symptoms such as inattention, impulsivity, or hyperactivity meet the criteria for Attention Deficit Hyperactivity Disorder (ADHD). The assessment usually includes a structured clinical interview, questionnaires, and a review of your medical and developmental history.

Who carries out the ADHD assessment?

ADHD assessments are conducted by experienced clinicians such as psychiatrists or specialist practitioners trained in diagnosing ADHD. They follow recognised clinical guidelines to ensure the assessment is thorough and evidence-based.

How long does the assessment take?

Our ADHD assessments take 2 hours, with a 30 minute follow-up appointment included in the fee to discuss the results.

Do I need to complete questionnaires before the appointment?

Yes. Prior to the appointment you will be asked to complete screening questionnaires. In some cases, questionnaires may also be completed by a parent, partner, or teacher to provide additional insights into symptoms across different environments.

Will I receive a report after the assessment?

Yes. After the assessment you will receive a comprehensive written report summarising the findings, whether diagnostic criteria are met, and recommendations for treatment or support where appropriate.

Is the report suitable for schools, universities, or employers?

Yes. The assessment report is professionally written and suitable to share with schools, universities, or employers where evidence of an ADHD diagnosis may be required to support reasonable adjustments or additional support.

What happens if I am diagnosed with ADHD?

If ADHD is diagnosed, the clinician will discuss treatment options and support strategies. This may include medication, behavioural strategies, coaching, or referral for additional support services.

What if I am not diagnosed with ADHD?

If ADHD is not diagnosed, the clinician will still discuss the findings and may suggest alternative explanations for your symptoms along with recommendations for further support if needed.

Do I need a GP referral?

No. In most private clinics you can book an ADHD assessment directly without a GP referral, although some patients choose to involve their GP in their ongoing care.

What information should I bring to my ADHD assessment?

It can be helpful to bring any relevant information about your symptoms, such as previous medical reports, school reports, or notes about how attention, focus, or impulsivity affect your daily life. If available, examples of how symptoms impact work, education, or relationships can also help the clinician build a clearer picture during the assessment.

Important Information

Sharing your care with the NHS

Before booking your appointment, please be aware that following a private ADHD assessment, if a diagnosis of ADHD is confirmed your NHS GP may not continue to prescribe ongoing medication through the NHS.

Shared care is an arrangement between you, your GP, and your private clinician that allows your ADHD care to be managed safely between both services once your medication is stable, after discussion and agreement.

While many GPs accept shared care to help ensure continuity of your treatment, they are not required to do so. If your GP is unable to enter into a shared care agreement your private clinician will continue to be responsible for prescribing and monitoring your ADHD medication. This will incur additional ongoing costs for the service and your medication at the pharmacy.
