D4 Driver Medicals
LGV/HGV/PCV Driver Medical Services in Cardiff, Newport & Swansea.
In order to drive an LGV (Large Goods Vehicle), HGV (Heavy Goods Vehicle) or PCV (Passenger carrying vehicle) you must apply for LGV/HGV/PCV licence.

When you first apply, you will be required to complete and submit a D4 form to the DVLA who will decide if you are medically suitable to drive large commercial vehicles. Before submitting the D4 form, it must first be completed by a qualified doctor, who will undertake a medical examination to report on your medical fitness and suitability.
Information: Please can you bring to your appointment a copy of your NHS GP Medical Record Summary Sheet. Please be aware if you wear glasses, you will also need to get an Optician to sign off the vision section of the D4 form (this can be done before or after your appointment with The IGP).

A Medical Examination Report (D4) can be completed by our doctors if you are intending to apply for or renew your driver's licence for any of the following:
A vehicle between 3.5 & 7.5 tonne or LGV (C, C1+E, or C+E)
A bus (PCV) over 8 passenger seats (D1, D1+E, D or D+E).
During the examination, the doctor will interview the candidate discuss past medical history (You will be required to obtain your NHS medical records summary prior to the appointment) and any potential current medical conditions which may impact the candidate's ability to drive safely. Once, this is completed the examining doctor will undertake a brief physical examination, which usually takes no more than about 30 minutes. The completed D4 form is then submitted to the DVLA for assessment.
Eyesight - Please be aware that recent changes to the form require that you have your eyes tested and the first part of the form completed by an optometrist/optician if you wear glasses for driving. Please arrange this prior to making an appointment with us.
Neurological Problems - Epilepsy, seizures, blackouts, memory problems, Parkinson's or Alzheimer's disease, narcolepsy and cataplexy, stroke, prior brain surgery, or any chronic neurological condition such as multiple sclerosis.
Mental Health - Depression, psychiatric issues, dementia and/or cognitive impairment.
Alcohol and Drugs - Indicators of chronic drug and alcohol use.
Existing medical conditions - Diabetes, Hearts conditions and/or sleeping disorders. You will be required to obtain your NHS medical records summary prior to the appointment
Once you have passed this medical and granted your licence, this entitlement to drive an LGV/HGV, minibus or bus will last until a driver's 45th birthday. After this time a driver will need to renew their entitlement every five years until they are 65. Those drivers over 65 are required to renew their licence every year.
The DVLA recommends that your completed D4 medical should be sent within 4 months of your medical. However, we can usually deliver this for you on the day of your appointment.
As a Licenced LGV/HGV/PCV driver, you are required to report any changes in your health to the DVLA, especially if you develop a medical condition that could potentially disqualify you from driving.
The D4 form is needed for your medical. A hard copy can be obtained directly from the DVLA or from your local Post Office prior to your appointment.
The INF4D leaflet provides information relating to your D4 medical examination. It contains advice for what is involved in the examination and should be read prior to your appointment to ensure that the D4 medical is appropriate and that you meet its requirements.
If you cancel your appointment 24 hours before it starts, then you will receive a full refund. However, If you cancel your appointment within the 24-hour period before the appointment is due, then you will be liable to pay the cost of the consultation. If you do need to reschedule please contact us as soon as possible.
