Our D4 Driver Medical examination is essential for obtaining or renewing your LGV, HGV, or PCV licence. Conducted by experienced and qualified GMC and NMC registered professionals, our service ensures that you meet all the medical requirements set by the DVLA. Our D4 medical service evaluates your medical fitness to drive LGV, HGV, or PCV vehicles. This involves a comprehensive review of medical history, current health conditions, and a physical examination by a qualified GP. Please bring your NHS GP Medical Record Summary Sheet and ensure the vision section of the D4 form is completed by an optician if glasses are worn for driving.
The D4 form is submitted to the DVLA to determine medical suitability. Drivers are required to report any changes in health to the DVLA to maintain compliance.

Bring your NHS GP Medical Record Summary Sheet and the D4 form. If glasses are worn for driving, have an optician complete the vision section.
You need a D4 medical every 5 years until age 65, then annually.
You can obtain the D4 form from the DVLA or your local Post Office.

If you cancel your appointment 24 hours before it starts, then you will receive a full refund. However, If you cancel your appointment within the 24 hour period before the appointment is due, then you will be liable to pay the cost of the consultation. If you do need to reschedule please contact us as soon as possible.
