Our D4 medical service evaluates your medical fitness to drive LGV, HGV, or PCV vehicles. This involves a comprehensive review of medical history, current health conditions, and a physical examination by a qualified GP. Please bring your NHS GP Medical Record Summary Sheet and ensure the vision section of the D4 form is completed by an optician if glasses are worn for driving.
The D4 form is submitted to the DVLA to determine medical suitability. Drivers are required to report any changes in health to the DVLA to maintain compliance.

If you cancel your appointment 24 hours before it starts, then you will receive a full refund. However, If you cancel your appointment within the 24 hour period before the appointment is due, then you will be liable to pay the cost of the consultation.
